Keeping your personal information up to date helps us provide safe, timely and accurate care.
If you change your name, address, telephone number, or any other key personal details, please let our Reception team know as soon as possible.
You can update your information by:
- calling the practice
- writing to us
- completing the online update form
Accurate contact information ensures we can reach you about appointments, test results, referrals and urgent clinical matters.
If You Move Home
If you move outside our practice boundary, you may need to register with a GP closer to your new address.
This is important to ensure:
- home visiting can be provided safely when needed
- you can access urgent care quickly
- your new GP can coordinate local services effectively
Your NHS medical card or NHS App may include guidance on what to do when moving to a new area, but our Reception team can also advise you on how to register with a new GP and ensure continuity of care.
Why Keeping Details Updated Matters
Up‑to‑date information supports:
- accurate medical records
- safe prescribing and medication reviews
- correct referral pathways
- appointment reminders and recall systems
- emergency contact accuracy
This is part of your responsibility as a patient, and helps us maintain high‑quality care and standards.
