What is a Primary Care Network (PCN)?

Beacon Medical Practice is proud to be part of First Coastal PCN, a network of practices along the Lincolnshire coast working together to meet the needs of our unique coastal communities.

PCNs were created to strengthen community based care, support prevention, and make it easier for patients to access the right professional at the right time.

What PCNs Do:

•Improve access — offering additional appointments, extended hours, and new clinical roles so patients can be seen sooner.

•Provide specialist services — such as enhanced health checks, structured medication reviews, frailty support, and personalised care planning.

•Support long term conditions — helping patients manage conditions like diabetes, heart disease, respiratory illness, and mental health needs.

•Join up local care — working closely with community teams, social care, voluntary organisations and hospitals to ensure smoother, safer care.

•Focus on prevention — delivering vaccination programmes, health promotion, early detection and proactive outreach.
