We are expanding our multidisciplinary team and we’re looking for a motivated, patient‑centred Pharmacist to join our friendly, forward‑thinking coastal practice.
You’ll play a key role in medicines optimisation, long‑term condition management, and supporting safe, effective prescribing across our three sites.
This role is ideal for a pharmacist who thrives in a collaborative environment, enjoys problem‑solving, and wants to make a meaningful impact on patient care within a supportive NHS primary care setting.
The Role includes:
- Medication reviews — structured medication reviews (SMRs), polypharmacy reviews, and medicines reconciliation
- Long‑term condition support — contributing to management of hypertension, asthma, COPD, diabetes and more
- Clinical support — assisting GPs with complex prescribing queries and safety monitoring
- Quality improvement — contributing to audits, QOF, IIF and medicines safety initiatives
- Patient education — supporting patients to understand and manage their medicines
- Collaborative working — working closely with GPs, nurses, HCAs, other multi-agency colleagues
You will need:
- To be a GPhC‑registered Pharmacist
- Have experience in primary care, community, hospital or PCN settings
- Have strong clinical reasoning and communication skills
- To be a proactive, organised, supportive team player
- To hold an Independent Prescriber qualification (or be willing to work towards it)
