We are expanding our multidisciplinary team and we’re looking for a motivated, patientcentred Pharmacist to join our friendly, forwardthinking coastal practice.

You’ll play a key role in medicines optimisation, longterm condition management, and supporting safe, effective prescribing across our three sites.

This role is ideal for a pharmacist who thrives in a collaborative environment, enjoys problemsolving, and wants to make a meaningful impact on patient care within a supportive NHS primary care setting.

 

The Role includes:

  • Medication reviews — structured medication reviews (SMRs), polypharmacy reviews, and medicines reconciliation
  • Longterm condition support contributing to management of hypertension, asthma, COPD, diabetes and more
  • Clinical support — assisting GPs with complex prescribing queries and safety monitoring
  • Quality improvement — contributing to audits, QOF, IIF and medicines safety initiatives
  • Patient education — supporting patients to understand and manage their medicines
  • Collaborative working — working closely with GPs, nurses, HCAs, other multi-agency colleagues

You will need:

  • To be a GPhCregistered Pharmacist
  • Have experience in primary care, community, hospital or PCN settings
  • Have strong clinical reasoning and communication skills
  • To be a proactive, organised, supportive team player
  • To hold an Independent Prescriber qualification (or be willing to work towards it)
